Job Description

We seek a licensed Home Health Physical Therapist to provide skilled, patient-centered care in private residences. Responsibilities include evaluating patients, developing individualized treatment plans, improving mobility, managing pain, preventing falls, and educating patients and families to promote safe, independent function at home.

• Qualifications: current PT license or DPT; home health experience preferred; valid driver’s license; strong clinical judgment, communication skills, and ability to work independently.
